Hidden Dangers of Over-the-Counter Pain Relievers - AARP

Drugstore shelves are overflowing with options for pain relief. There are packages of pills, patches, creams and liquids — all designed to alleviate everyday aches and injuries.

And older adults are the largest consumers of these over-the-counter (OTC) medications, research shows. About half of Americans ages 50 to 80 take two or more nonprescription medications, including supplements, a 2020 report from the University of Michigan's National Poll on Healthy Aging found.

But just because something is sold without a prescription doesn't mean it's risk-free, says Amy Thompson, associate chair in the Department of Clinical Pharmacy and clinical associate professor at University of Michigan's College of Pharmacy. “And those risks heighten as we age,” she adds.

Here's what you need to know before you pop an OTC pain reliever.

Acetaminophen

Also known by the brand name Tylenol

Acetaminophen is one of the most commonly used drugs in the U.S. and is the top recommended OTC pain relief option for most older adults. It can be used to ease headaches and muscle aches and to reduce fever. It's also good for treating osteoarthritis — the type of arthritis that comes from everyday wear and tear, says Ashley Garling, clinical assistant professor at the University of Texas at Austin's College of Pharmacy.

But because too much acetaminophen can cause liver damage, certain people should reduce their dosage or avoid it altogether. Talk to your doctor before taking acetaminophen if you have liver disease or drink three or more alcoholic beverages per day. (Alcohol causes the liver to convert more of the acetaminophen into toxic byproducts; if these toxins build up, damage can result.)

Otherwise, Garling says “as long as your overall daily dose is under 3,000 milligrams, [in a 24-hour cycle], it's usually fairly safe.” Taking a higher dose than recommended will not provide more relief, but can be dangerous, the Food and Drug Administration (FDA) says.

The danger is often disguised

But keeping your daily dose under 3,000 milligrams per day can be tricky, especially since acetaminophen shows up in hundreds of other medications, including OTC cold and flu relief (Theraflu and Sudafed are examples) and allergy medicines.

"And so people might be taking Tylenol for their knee pain and then taking NyQuil for a cold, not realizing there's Tylenol in both of them. They could easily be exceeding the daily dose without even realizing it,” Thompson says.

Be sure to pay close attention to the label, as overdosing on acetaminophen is the most common cause of drug-related liver injury. Acetaminophen isn't always spelled out; abbreviations such as APAP, Acetaminoph, Acetaminop, Acetamin or Acetam may be used instead, the FDA says.

What to Know About ‘PM’ Pain Relievers

Older adults should avoid the nighttime version of any pain reliever, says Hillary Lum, a geriatrician and assistant professor in the Division of Geriatric Medicine at the University of Colorado School of Medicine. These medicines often contain diphenhydramine, an antihistamine that can aid sleep — thus the “PM” that's often in the product name indicating it should be taken at night — but they can also cause falls, confusion, blurred vison and urinary problems in older adults. In addition, diphenhydramine has been linked in studies to increased dementia risks. If a nighttime pain reliever is being used primarily as a sleep aid, talk to your doctor about alternatives.

Nonsteroidal Anti-Inflammatory Drugs (NSAIDs)

Also known as ibuprofen (brand names Advil, Motrin) and naproxen (Aleve)

NSAIDs are another class of pain relievers sold over the counter. They're helpful at reducing pain caused by inflammation — like arthritis or an athletic injury — and can also aid with fevers and mild pain from headaches and muscle aches.

But NSAIDs are not recommended for everyone, and older adults are often advised to avoid them, or at least use them with caution. That's because they can come with a list of increased risks for people age 50-plus.

Bleeding, heart problems and other hidden dangers

Use of NSAIDs can cause bleeding in the stomach or in the digestive tract, which can come on without warning signs and can be serious. People who take NSAIDs every day or regularly are at increased risk for these adverse events, according to the FDA. The risk is also higher for adults older than 65, people with a history of stomach ulcers, and people who take blood thinners like warfarin or corticosteroids, such as prednisone.

Adults with blood pressure problems should know that NSAIDs can raise blood pressure. And all NSAIDs, except for aspirin, can increase chances of heart attack or stroke, even after the first few weeks of use. People who have cardiovascular disease are at highest risk.

NSAIDs have also been linked to kidney injury in older adults, “which is something I really worry about, too,” Thompson says. “As we age, we really depend on [lipids in the body called] prostaglandins to keep the blood flow to our kidneys. And the way [NSAIDs] work is they inhibit those prostaglandins,” causing the blood vessels to constrict, which can result in kidney injury.

Finally: NSAIDs carry the risk of potential skin reactions, the FDA warns. Patients should be on the lookout for symptoms of skin reddening, rash or blisters.

"So you really want to leave [NSAIDs] as kind of your last choice, which is interesting, because most people go to these first. But when you're over 50, this one needs to start moving down the list, versus something like a Tylenol,” Garling says.

What to Know About Aspirin

Aspirin is an NSAID, but it's generally not recommended for pain relief in older adults because it can cause bleeding in the stomach and the brain — and the bleeding risk increases with age. Plus, other OTC remedies are more effective at mitigating pain, both Garling and Thompson say.

Where aspirin can come in handy is as a preventive therapy for people who have cardiovascular disease or who have already had a heart attack or stroke. Regular use can reduce risks of another cardiovascular event by keeping the blood from clotting and increasing blood flow to the heart and brain. It is not recommended to prevent first-time heart problems and is often misused as such.

Always check with your health care provider before starting aspirin therapy to weigh the benefits and the risks and to make sure any OTC and prescription medications you're taking are OK alongside aspirin — otherwise “you could be doing your body more harm than good,” the FDA says.

Topical pain relievers

Not all pain relievers come in a pill. Topical medications — gels, creams, patches and sprays — can be effective at targeting pain, especially local pain in muscles and joints. Plus, if you're worried about side effects from pills, topical pain relievers carry less of a risk “because there's less systemic absorption,” Thompson says.

  • Topical NSAIDs: Some OTC creams contain NSAIDs, including diclofenac sodium (brand name, Voltaren), which is a popular and effective medicine for treating arthritis pain. The typical side effects of NSAIDs are less of a concern with topicals, but still need to be considered, especially for people with a history of stomach and heart issues.
  • Lidocaine and menthol: A key ingredient in other topical pain relievers is lidocaine. It provides a numbing effect that can “really help with that more acute, stabbing pain sort of feeling,” Garling says. Menthol, which has a cooling sensation, is another common active ingredient in pain-relieving creams and sprays.
  • Capsaicin: If you're a fan of spicy foods, you may be familiar with capsaicin, since it's the compound that gives hot peppers their kick. But it also helps block pain signals in the body and can be a safe and effective way to relieve joint and muscle pain, as well as neuropathy, or pain from nerve damage in the hands and feet, often experienced by diabetics, Garling says.

    Just be sure to wash your hands after you rub it on your skin — you don't want capsaicin getting in your eyes. And don't give up on it too soon, Garling says. “The biggest caveat to this one is that you do have to use it regularly. ... Sometimes it takes up to two weeks before you notice a huge difference.”

One downside to topical pain relievers: They often cost more than their oral counterparts, Thompson says. A 5.3-ounce tube of Voltaren, for example, is $28, whereas a bottle containing 50 ibuprofen pills is about $5.

Talk to your doctor and pharmacist

The key takeaway to OTC pain medication is to keep your health care provider in the loop, says Hillary Lum, a geriatrician and assistant professor in the Division of Geriatric Medicine at the University of Colorado School of Medicine — especially “given that there are now more and more prescription-strength medications that are over the counter and the package inserts are really difficult to understand.”

Her advice? Bring a list of everything you are taking, “including your supplements and your over-the-counter medication” to your provider at least once a year. Don't forget to add any CBD (cannabidiol) use to this list, Lum says. Oils, lotions and gummies containing CBD have become prevalent, especially as an alternative pain reliever, and just like any other OTC substance, your doctor should know if you're using it.

And remember: If you're stumped in the medicine aisle, check in with the pharmacist for guidance. “We're the only ones that you can walk up to in your CVS and your local drugstore and talk to within seconds,” Garling says. “And if we're back there behind the computer, don't be afraid to ask for us — because we actually love questions.”

DUBAI, June 30 (Reuters) - More than 500,000 firms in the United Arab Emirates (UAE) must disclose their ultimate owners from Wednesday or face penalties, as the Middle East financial hub tries to avoid inclusion on a dirty money watchlist.

However, under legislation passed last year beneficial and legal ownership data will only be disclosed to the government and not publicly accessible. Financial transparency advocates say having that data available to all is the gold standard, although so far only a handful of countries do so.

The Financial Action Task Force (FATF), an intergovernmental anti-money laundering monitor, declined to comment on the UAE law but directed Reuters to a 2020 report which said "fundamental and major improvements" were needed to avoid it placing the UAE on its 'grey list' of countries under increased monitoring.

Countries on this list risk reputational damage, trouble accessing global finance and increased transaction costs.

The UAE has tightened up over the last few years to overcome a perception it is a hot spot for illicit money and in February the government created an Executive Office for Anti-Money Laundering and Counter Terrorism Financing.

The Middle East and North Africa Financial Action Task Force (MENAFATF) recently re-rated the UAE's level of transparency and beneficial ownership from 'partially compliant' to 'largely compliant', Amna Fikri, the foreign ministry's economic and trade director, said in response to questions about what will happen to the information once it is collected and who will be able to search it.

The UAE's economy ministry has said that 513,000 companies across 38 licensing authorities must have submitted registers on their beneficial owners and shareholders by Wednesday or face penalties.

These include a written warning, a 100,000 dirham fine, license suspension or restrictions on the powers of the board.

"The risk of criminals being able to misuse legal persons in the UAE for money laundering/terrorist financing remains high, particularly through concealment of beneficial ownership information via complex structures or the use of informal nominees," the FATF's 2020 report said.

'CONFIDENTIALITY'

The past decade has seen a global shift towards ownership transparency to discourage tax evasion, money laundering and terrorist financing.

The UAE government said in response to the Reuters report that it "takes its role in protecting the integrity of the global financial system extremely seriously. This means actively administering and enforcing economic and trade sanctions."

And sources told Reuters last year that an initiative involving 11 countries by the London Bullion Market Association (LBMA) to improve regulation had been driven by concerns over the UAE's gold industry in particular.

The UAE's Ministry of Foreign Affairs and International Cooperation said in response to that report that it recognised the importance "of developing increasingly robust mechanisms to address the challenges brought about by financial crime".

Different levels of accessibility and information verification can make jurisdictions more or less discouraging to companies wishing to hide ownership structures, campaigners say.

"Centralised beneficial ownership registers open to the public are preferred, to preserve trust in the integrity of business transactions and the financial system," MaĂ­ra Martini, an expert on corrupt money flows at Transparency International.

More than 80 countries have so far adopted beneficial ownership registry legislation, but not all are operational and only a few are public.

UAE ownership data will be collected by each licensing authority and ultimately held in the National Economic Register by the Ministry of Economy, which said last month it would be handled with confidentiality and not for commercial purposes.

"Even the employees of these government entities do not have free access to this data except in cases of investigation and the required disclosure to specific official entities, according to strict internal policies and regulations," it said.

Martini said it was important to follow up on how the registry measures are implemented.

"For the UAE it is important to create a centralised register that brings together information from all the different registers, making sure those registers are collecting the same type of data and using the same system so there is no arbitrage."

SEAL BEACH (CBSLA) — A new sticker being offered free by the Seal Beach Police Department may help save lives.

The new sticker, which is designed to be placed on or near a home’s front door or on car windows, are intended to alert law enforcement officers that a person inside the home or vehicle may have special needs and may not easily understand instructions.

Seal Beach police officers are trained regularly on how to interact with people who have special needs, Chief Philip L. Gonsak said.

“With that, the goal of this sticker is to assist us with those calls for service involving anyone who has them and we ask for your partnership in identifying your loved one’s special needs beforehand,” Gonshak said in a statement.

KAILUA-KONA — The Marine Mammal Center admitted an endangered Hawaiian monk seal from Papahanaumokuakea Marine National Monument at its hospital in Kailua-Kona.

The successful rescue and transport of PP08, a prematurely weaned female pup, was made possible thanks to the center’s partnership with the National Oceanic and Atmospheric Administration, the U.S. Fish and Wildlife Service and the U.S. Coast Guard.

“Separated from her mom too soon, this young pup had nearly zero chance of survival without intervention,” said Dr. Sophie Whoriskey, Hawaiian monk seal conservation veterinarian at The Marine Mammal Center, in a press release. “Each individual matters, especially for an endangered species, and we are grateful to our partners for their support in achieving our mission, and ensuring this young pup has a second chance at life.”

Since late May, biologists from USFWS had been monitoring PP08 on Midway Atoll National Wildlife Refuge and Battle of Midway National Memorial.

Born in mid-April, PP08 weaned from her mother too early and was observed underweight, a major risk for a female seal in an endangered population.

Based on USFWS’s observational reports, NOAA’s Hawaiian Monk Seal Research Program decided a rescue and transport of PP08 to The Marine Mammal Center’s hospital in Kona was necessary for rehabilitative care until she can be released back to the wild at a healthy weight.

“PP08 was measured at just 78 cm girth, and what we’ve seen is that pups who wean at less than 90 cm girth have essentially no chance of surviving to the next year,” said Dr. Michelle Barbieri, lead scientist for NOAA’s Hawaiian Monk Seal Research Program.

“Thanks to the combined efforts that went into detecting, rescuing and transporting PP08 to definitive care at The Marine Mammal Center’s hospital, especially before she became too depleted or sick, she has a very good chance at being fully rehabilitated and released back in Papahanaumokuakea, where she’ll have a second chance at growing to adulthood.”

NOAA coordinated logistical and operational support for this rescue, in partnership with USCG, USFWS and the center. Earlier this month, a U.S. Coast Guard C-130 aircraft transported Whoriskey and her team to Midway Atoll to accompany PP08 on her flight to Kona.

“One of the Coast Guard’s mission sets includes the protection of living marine resources,” said Lt. Sam Potter, a USCG pilot at Air Station Barbers Point on O‘ahu involved in the rescue. “It is a very fulfilling mission, and we are proud to assist our partner agencies in recovery efforts of these endangered animals.”

During her initial critical-care period, center experts noted that PP08 was moderately malnourished, but is alert and active. The center’s veterinary team took a series of blood samples and a fecal sample for further analysis during the animal’s admission exam.

“PP08 has been feisty and extremely vocal since her arrival, which is a really positive sign for a young pup,” Whoriskey said. “She is currently being tube-fed a mix of electrolytes and sustainably-ground herring to help boost her weight. Our team plans to introduce her to whole herring and offers of live fish in the coming days to help further spur her appetite and get the vital nutrients she needs.”

Since 2014, the center has rehabilitated and released 35 monk seals, most of which have been rescued from and returned to Papahanaumokuakea as part of their partnership with NOAA, utilizing resources in the monument to identify seals in need, rescue and rehabilitate them, and give them a second chance at life.

The center’s partnership with NOAA and other cooperating agencies is more important than ever to prevent this endangered species from becoming extinct, the release continued.

Approximately 30% of monk seals that are alive today are alive due to conservation efforts led by NOAA and partners like The Marine Mammal Center.

The Hawaiian monk seal is one of the most endangered seal species in the world.

It’s endemic to the Hawaiian Islands. There are approximately 1,400 left in the wild, and as many as 900 of them reside in the northwestern Hawaiian Islands. The rest of the population has taken residence in the main Hawaiian Islands, Oahu, Big Island, Kauai, Maui, Molokai, Lanai, Kahoolawe and Niihau. The Hawaiian monk seal was hunted to the brink of extinction in the late 1900s, but thanks to massive conservation efforts from NOAA (National Oceanic and Atmospheric Administration) and Hawaii Marine Animal Response (HMAR) the population has seen a slight increase in recent years. 

Currently the Hawaiian monk seal suffers greatly from human pollution, entanglement in fishing gear and the ingestion of plastics, which commonly collect in the Northwestern Hawaiian Islands due to drifting from the main islands. They also suffer from habitat loss due to rising oceans and eroding atolls, this forces the pinnipeds to find different places to haul out and raise their pups. 

A Hawaiian monk seal usually reproduces at the age of five or six years old. They will stay on land with them for around six weeks. During this period the mother will not eat or leave her pup. She must teach the pup everything it will need to survive on its own. After six or seven weeks, the mother will leave and the pup is left to fend for itself.  Pups are born at around 30-40lbs and will gain an average of around 5lbs a day. When it’s time for the mother to leave a healthy pup, it will weigh between 150-200lbs! Pups are born with black fur, but will change to a silvery colour usually by the time their mother leaves. Throughout their lives they will go through an annual shedding cycle called catastrophic molt. During this period of time, they will stay on land for around 10 days. A Hawaiian Monk seal can live for 25 to 30 years although it is very rare to find individuals who will live to this age in the wild.

Last season, with Jones, Matt Ryan only cracked the weekly Top 12 QB finishers six times while finishing outside of the Top 18 at his position on seven occasions.

Jones’ departure doesn’t only limit Ryan’s upside. The writing was on the wall late last year. His offensive took a turn for the worst down the stretch in 2020. From Week 11 on, the Falcons’ franchise QB was sacked 22 times, second-most in the NFL! In addition, opposing defenses exposed the Atlanta line utilizing the blitz 108 times in those final seven weeks, most in the league. Don’t be fooled by Matt Ryan’s volume, either. He led the league in completions and attempts in 2020 but still finished just 15th in fantasy points per game. Leave Ryan for someone else.

Superflex Leagues

Ben Roethlisberger (QB – PIT) ECR QB22 | ADP QB17
Ben Roethlisberger’s overall 2020 fantasy numbers aren’t terrible, certainly worthy of a selection as your second QB in Superflex leagues, right? Wrong. Over the final six weeks of the season, Roethlisberger was putrid. He averaged 16.3 fantasy points per game (25th amongst QBs) while “airing it out” at just 5.6 yards per attempt (worst in the NFL). Yes, there are plenty of weapons for the Pittsburgh signal-caller, but his downfield passing skills are long gone.

In front of Roethlisberger, the Steelers have lost essential cogs. Maurkice Pouncey retired, Alejandro Villanueva signed with Baltimore, and David DeCastro was released. Exposing a 39-year-old to the inconsistencies that come with an offensive line learning to gel is not ideal. Especially since Ben Roethlisberger has nursed over a dozen injuries during his career, he’s a Hall of Famer, but a 2021 hard pass all the same.

Derek Carr (QB – LV) ECR QB23 | ADP QB20
A theme is developing in this article: Avoid QBs with Offensive Line Concerns. Derek Carr is another passer whose linemen will be learning on the job in 2021. The Raiders traded three veteran trenchmen for draft picks and did not sign any big free agents to replace them, leaving Carr protected by an entirely different group. In addition, his mobility isn’t a strength, so any leaks upfront will end with the QB on his back.

Carr’s not going to win any fantasy weeks. He never has. Mike Tagliere writes yearly about “Boom and Busts,” and the Vegas franchise QB rarely lands a boom-worthy score (just once under Jon Gruden). Between 2019 and 2020, Derek Carr has thrown one, or no touchdown passes in 50% of his games. So I wouldn’t even recommend him as a bye-week filler.

Best Ball Competitions

Tua Tagovailoa (QB – MIA) ECR QB21 | ADP QB21
Speaking of boom/bust candidates, Tua Tagovailoa imploded for fantasy managers in 2020 60% of the time. He finished outside the Top 18 quarterbacks six times and inside the Top 12 only twice over ten games played. In addition, the Dolphins offense didn’t trust Tua in scoring situations. He only had 3.4 attempts per game in the red zone, making it difficult for him to have a monster week.

Best Ball teams need weekly upside, and Tagovailoa hasn’t shown that in the NFL yet. Miami drafted his former collegiate teammate Jaylen Waddle (WR – MIA) and added Will Fuller (WR – MIA) to the mix in hopes of helping their young QB become the playmaker they believe him to be. Unfortunately, the latter can’t stay healthy, and the former will have to adjust to the NFL quickly. Tua will face some stout defenses in the division once again. I would much rather back up my Best Ball QB1 with a counterpart who has “boomed” before, like Baker Mayfield (QB – CLE) or Deshaun Watson (QB – HOU). I am playing for those big weeks, not waiting to see what could be.

The Navy will “not pursue further action” against retired SEAL Eddie Gallagher after he said during a May podcast that “we killed that guy,” referring to an Islamic State captive.

“The Navy reviewed the matter and will not pursue further action. Gallagher’s statements were not corroborated, nor does substantive information exist to merit an investigation based on those statements,” Navy spokeswoman Cmdr. Courtney Hillson told The Hill on Tuesday.

Hillson added that evidence against Gallagher related to medical treatment and the death of the detainee “was already investigated and/or adjudicated at Gallagher’s court-martial.”

Under the Uniform Code of Military Justice and the Constitution, “he cannot be prosecuted again for these alleged offenses,” the spokeswoman said.

Politico first reported on the decision.

Gallagher in 2018 was charged with using a hunting knife to murder Khaled Jamal Abdullah — a 17-year-old ISIS captive who was already injured, among other charges stemming from a 2017 deployment to Iraq.

But a member of Gallagher's team testified that he killed the prisoner instead of Gallagher, leading to his acquittal.

Gallagher was instead convicted of one charge of posing with the captive’s body, and the Navy demoted him from chief petty officer to petty officer first class.

Then-Navy Secretary Richard Spencer was fired amid the fallout from Trump’s decision to reverse the demotion.

But on a May 4 episode of “The Line” podcast, Gallagher said he and his fellow SEALs intended to practice medical procedures on the dying captive and that they had all agreed to the idea.

“The grain of truth in the whole thing is that that ISIS fighter was killed by us and that nobody at that time had a problem with it,” Gallagher said.

“We killed that guy. Our intention was to kill him. Everybody was on board,” Gallagher added. “It was to do medical scenarios on him until he died.”

RICHMOND, Va. (WWBT) - On Thursday, Virginia will legalize simple possession of marijuana, allowing adults 21 and up to possess up to one ounce of pot for personal use. Virginia Governor Ralph Northam believes this legalization will provide an opportunity to further criminal justice reforms around the Commonwealth.

Under the state’s Cannabis in Virginia website, the state says the law will also lay out a timeline for state agencies to seal marijuana-related records, but it could take time.

Starting July 1, 2021, the state says all records of misdemeanor possession with intent to distribute marijuana arrests, charges, and convictions will be automatically sealed from public view in the Virginia State Police’s systems.

This follows the sealing of simple possession of marijuana records, which the state says were completed in 2020.

By July 1, 2025, all records of arrests, charges, and convictions for simple possession of marijuana or misdemeanor possession with intent to distribute will be automatically sealed across all state and private databases. This is will happen once record-keeping systems are updated among several state entities.

“Sealing a record really creates a benefit for the collateral consequences such as housing, pell grants, education, employment,” said Chelsea Higgs Wise, a local activist. “The sealing records does not stop the criminal justice system, the cops, the prosecutors, the judges from seeing your record and it being used against you later.”

During the marijuana bill signing in April, Virginia Governor Ralph Northam said the legalization was about “equity” above all as studies show Black Virginians are four times more likely to face marijuana charges.

“Making sure this is a way to bring racial justice to the criminal justice system,” said Cynthia Nwarache, an advocacy coordinator with OAR of Richmond.

The organization helps inmates behind bars know their rights while navigating the criminal justice system. They also help people re-enter society after serving their time.

Nwarache hopes this legislation will offer second chances.

“Paired with a record and being a person of color, that further causes employers, for housing, for landlords, to further discriminate against someone from renting to them or offering a job position,” she said.

Under the Cannabis in Virginia website, people will also have the ability to petition a court to seal all other marijuana-related misdemeanors and most marijuana-related felonies.

Wise also adds that there’s still more work to be done to pass laws about expungement.

Video: Great White Shark feasts on seal ‘close to shore’ in Provincetown - The Boston Globe

As a Provincetown native well-acquainted with Cape Cod’s many beaches, Dom Richmond has seen a lot of sharks over the years, including blue and basking.

But at age 30, there was one type he hadn’t checked off his list: a great white. That changed on Saturday — and he captured the exciting moment on video.

While sitting on the beach, Richmond and his girlfriend, Susy, noticed a seal bobbing its head not too far from shore at Race Point Beach. He didn’t think much of it at first, but then Richmond, who lives in Brooklyn but is home visiting his family in Provincetown, noticed that the animal was “swimming awkwardly.”

Moments later, he saw the dorsal and tail fin of a great white shark cutting through the ocean, closing in on the clearly injured pinniped.

“I’ve been wanting to see one for a long time,” Richmond said. “Every time I go on the water I’m always looking for sharks.”

Richmond, a photographer and videographer, said the seal first swam toward shore, where it rested for a short time before returning to the water.

“It was kind of rolling in with the waves and trying to just lay up on the beach,” he said. “It got to the beach and was sitting there for awhile.”

At that point, the shark had traveled down the beach, where it was spotted by other people and a lifeguard. This drew a small crowd of people to Richmond and the seal.

“Once the lifeguard started coming down the beach, more people followed,” Richmond said.

When the seal ventured back into the ocean, the shark made a pass at its prey in the shallow waters. This time, the seal did not escape.

“The shark would come at it, but at first it was too close to shore so it swam away again,” Richmond said. “Ten minutes later the shark came back, and that’s when you see the shark take the seal under and it never came back up.”

Richmond posted the video to his Instagram account, Richmond Vision Studio, on Monday.

“Great White shark comes way [too] close to shore while eating a seal!” Richmond wrote in the caption for the video, which he paired with dramatic, “Jaws”-like music.

In a longer version of the video sent to the Globe, the shark can be seen circling around the seal, its tail thrashing in the water.

According to the Atlantic White Shark Conservancy’s Sharktivity app, which provides reported and confirmed shark sightings, a temporary no-swimming order was issued by lifeguards at Race Point Beach just before noon Saturday, after the shark was first spotted. The order expired one hour after the sighting.

Officials from the conservancy, a nonprofit that works with state officials to tag and track great whites off Cape Cod, confirmed Tuesday that the shark in the video was a great white.

Another great white shark was spotted off Orleans Beach on Sunday, according to the conservancy, where it ate a seal “a couple hundred yards off the beach.” A shark was also spotted off Chatham later that afternoon by a pilot.

There were two other sightings last week that temporarily closed Head of the Meadow Beach in Truro.

Richmond said the video didn’t “do justice” to showing how close to shore the shark had come for its meal. He said the apex predator was roughly 20 to 30 feet away from where the waves were breaking on the sand.

“It was something I had been trying to see for as long as I’ve known about sharks on the Cape,” said Richmond. “For me to see it on the beach at my feet, I just happened to be at the right place at the right time.”

UPDATE: Letters of interest sought for Town Seal Review Committee - Community Advocate

UPDATE: The deadline to submit letters of interest for the Town Seal Review Committee has been extended to July 16. 

WESTBOROUGH – Those interested in joining Westborough’s soon-to-be-formed Town Seal Review Committee can still submit letters of interest to the Town Moderator. 

Established by Town Meeting vote earlier this year, the committee will review the history of town seals in Westborough, consider whether to replace Westborough’s current seal and provide a report to a future town meeting based on that deliberation. If it suggests changing the seal, the committee will be further tasked with laying out a possible process for making that happen. 

The original deadline for letters of interest has been extended. Originally set for June 30, Town Moderator John Arnold announced in a new statement, June 28, that he will continue to accept letters through July 16.

This, he wrote in a message to the Community Advocate, is “due to end of school year activities and people’s busy schedules for the July 4th weekend.”

The town seal has been a topic of intense debate in town particularly over the last two years. 

The seal features a depiction of the cotton gin whose inventor, Eli Whitney, once lived in town. As a series of protests and social movements regarding racism in America have prompted a reexamination of historical symbols and figures, some have discussed the ways Whitney’s invention helped industrialize systems of slavery in the American South. 

As a result, critics say Westborough should remove the cotton gin from its seal.

With others in town arguing the seal should remain unchanged, the matter will, for now, be in the hands of the Town Seal Review Committee before ultimately coming back before Town Meeting.

Former U.S. Navy SEAL Edward Gallagher, acquitted in a 2017 war crimes trial but later charged with posing with an Islamic terrorist's corpse, joined "Hannity," along with his wife Andrea to discuss his case and a new book the couple wrote that includes QR-code links to actual courtroom audio.

Gallagher told host Sean Hannity that the book – "The Man in the Arena" – intends to allow the public to find out the truth for themselves and make their own decisions about his case, after he was vilified in the media and by many on the left.

"The reason we wrote the book is to get the truth out there about what happened and what took place during that whole two-year ordeal, the media decided to portray me in one way, made me look like a psychopath, a warmonger for actually wanting to do my job," he said.

"[They] demoniz[ed] me, demonized all my friends and brothers that have fought over the past 20 years so I wanted to get the truth out there and just show the public that these people cobbled together a case out of a bunch of white lies which then escalated to even bigger lies so the prosecutors could try to get a win which 50% of that was not reported by the media."

The QR codes embedded in the book will link to courtroom audio, interviews and other evidence for the reader to make their own judgment.

Gallagher was ultimately found not guilty of the greater charge of indiscriminately firing at civilians in Mosul.

As Hannity noted, Gallagher was one of many but few who put their lives on the line for the good of the American people, and that service members like him should be treated the way the media treated him.

"The country owes you an apology. That shouldn't happen to our brave heroes, our national treasure willing to fight these wars, they send you into these war zones with handcuffs on, that's madness," the host said. "We can't fight wars this way ever again."

After his conviction, his punishment included a reduction in rank from chief petty officer to 1st class petty officer, which would have cost him about $200,000 in retirement funds. His family and defense team were fighting to have his rank restored.

However, in 2019, then-President Donald Trump issued an executive fiat calling for his E-7 rank to be reinstated.

"There are no words to adequately express how grateful my family and I are to our president, Donald J. Trump, for his intervention and decision," Gallagher said in a public statement following the former president's reversal. "We would also like to thank the American people for their unwavering support during this very difficult time for my family and I -- we can never thank you enough."

Former President Donald Trump has focused his anger over his election loss against top Republicans, including Senate Minority Leader Mitch McConnell and even a member of his own administration.

Trump on Monday turned his ire against McConnell and former Attorney General Bill Barr, claiming the two allowed "radical left" Democrats to "flood the system with mail-in ballots that are very open to fraud," as Trump described last November's election.

Trump also blasted Barr as a "RINO," or Republican in name only, and claimed he "went out with a whimper" just hours after Barr was quoted in an interview about his December 2020 "break" with his former boss.

Trump attacked McConnell as well, even though the then-Senate majority leader had voted against the president's second impeachment. Trump said the Kentucky Republican "did nothing" to help him overturn President Joe Biden's victory.

In a Sunday statement, Trump accused Barr of hypocrisy and of showing a lack of respect toward him. He asked, "If [Barr] felt this way, why did Barr say he was 'greatly honored' and 'proud to have played a role in the many successes and unprecedented achievements you have delivered for the American people' in the final letter he wrote to me? He said, 'Few could have weathered these attacks, much less forge ahead with a positive program for the country,'" Trump wrote.

In an Atlantic interview published Sunday, Barr said he enraged Trump after publicly declaring the Justice Department had not seen any evidence of presidential election fraud, particularly "on a scale" claimed by Trump and his allies.

The article, titled "Inside William Barr's Breakup With Trump," said the former attorney general "has been widely seen as a Trump lackey who politicized the Justice Department. But when the big moment came after the election, he defied the president who expected him to do his bidding."

Trump's Monday statements ridiculed both McConnell and Barr for refusing to overturn the election.

"Had Mitch McConnell fought for the Presidency like he should have, there would right now be Presidential Vetoes on all of the phased Legislation that he has proven to be incapable of stopping," the former president wrote.

"Not to mention, he lost two Senatorial seats in Georgia, making the Republicans the Minority in the Senate," Trump continued. "He never fought for the White House and blew it for the Country. Too bad I backed him in Kentucky, he would have been primaried and lost! Based on press reports, he convinced his buddy, Bill Barr, to get the corrupt (based on massive amounts of evidence that the Fake News refuses to mention!) election done, over with, and sealed for Biden, ASAP!"

Newsweek reached out for comment to representatives for Trump as well as McConnell and Barr but did not hear back before publication.

Rhabdo is rare but potentially fatal. Here’s why fitness experts fear a rise in cases this summer. - The Washington Post

This spring, when Justin Urban, 41, a slender hotel waiter and covid-19 survivor from Falls Church, Va., reentered his condo building’s gym for his first workout in months, he committed to nothing more than five minutes of power walking on a treadmill. His caution was the result of what happened to him in 2018, the last time he hit the gym after a long hiatus: He was hospitalized for a week after completing 40 negative pull-ups.

 Urban’s symptoms included stiff, sore elbows and a bout of brown urine (“the color of Coke,” he said). He was diagnosed with a rare condition called rhabdomyolysis (frequently shortened to rhabdo). Rhabdo results from overexertion typically brought on by repetitive, unfamiliar movement that stresses muscle cells to the point at which they essentially burst and die.

 The cells then leak their toxic contents — especially creatine kinase (a muscle enzyme) myoglobin (a muscle protein) — into the bloodstream, with potentially fatal results. A National Institutes of Health review found that as many as 40 percent of rhabdo cases end in kidney failure.

The primary diagnostic tool for rhabdomyolysis is a blood test for creatine kinase. Urban’s results showed of more than 100,000 units per liter, an amount that risked causing kidney failure and either death or lifelong dialysis (normal levels of the enzyme range from 22 to 198 units per liter). His urine was not tested, but the discoloration was likely the result of the excess myoglobin. He was put on intravenous saline to wash out the toxins.

“It was just kind of a shocking thing,” he said. “Forty pull-ups put me in the hospital for a week.”

There are roughly 42,000 cases of rhabdomyolysis a year among U.S. adults, but experts worry that the condition has found a fertile moment this summer among Americans experiencing weight gain. According to one 2021  study, pandemic stay-at-home orders led to a decline in physical activity. Another found that participants had gained an average of 1.5 pounds per month that they sheltered in place.

 As people continue to return to gyms and broader daily life, the race to lose that weight has intersected with both beach body pressure and a synchronous kind of national high school reunion vibe in which the newly chubbier among us are eager for friends and colleagues to see us as we were pre-pandemic. A nation where 74 percent of adults are already overweight is now writhing in the triangulation of these stressors this summer — and high-body temperature is a risk factor for rhabdo.

“We survived the pandemic because our bodies are built for survival,” said Ali Register, 40, a personal trainer for 12 years who is now the training director at the Balance Gym on Capitol Hill. “But for humans that usually means doing as little work as possible and consuming as much energy or calories as possible. Our bodies like us fat and lazy. We learned to run to escape danger, not to get fit.”

He detailed what he called a recent, major “tilt” among gymgoers: “Nine out of ten people now, they come in and the first thing they say to their trainer is something about their pre-pandemic body. That’s the standard. That’s the urgency. There’s a desperation to look better, and almost to be unaffected by the pandemic and everything we’ve been through.”

 That psychology can be amplified by gyms, which offer physical extremes usually unattainable during home or outdoor workouts, and group classes, where participants can frequently push past personal fatigue levels in the frenzy to keep up with everyone else.

At one point, CrossFit gyms displayed an “Uncle Rhabdo” mascot, and, in 2008, a Virginia man won a $300,000 settlement against his gym for a CrossFit-style workout that resulted in rhabdo.

 While extreme workouts are ripe for rhabdo, cases have occurred after more conventional exercise, consider the woman hospitalized after one 45-minute spinning class, the police officer who died from a 1.5-mile run and the hospitalized college football players in Iowa and Nebraska.

The Centers for Disease Control and Prevention maintains a list of contributing risk factors, including alcohol, antibiotics, antidepressants, caffeine, cocaine, cholesterol-lowering drugs called statins, cold and allergy medicines, methamphetamines, and supplements including creatine and ephedra. Because of sickle cell disease, Black people are also at higher risk of rhabdo. Men, who tend to have more muscle mass, are more at risk than women.

 “I’m worried that there might be a spike with people going back into it with the reopenings,” said Maureen Brogan, associate professor of medicine at Albert Einstein College of Medicine in New York and a nephrologist at Montefiore Health System. “I feel like I’m starting to see younger. I’ve seen 18-year-old kids who get it. A lot are 30 or less,” said Brogan, who published a 2017 study on rhabdo cases in the American Journal of Medicine.

 Part of the problem, she explained, is that rhabdo flourishes under the kinds of fads and novelties upon which gyms and fitness purveyors rely. “It could be from any repetitive motion of a muscle when the muscle has not been used in that way,” Brogan said. “It can be lots of sit-ups. It can be anything.”

She added that she has even seen rhabdo cases result from casual activities such as gardening, house painting and shoveling snow. And she warned that nonsteroidal anti-inflammatory drugs such as ibuprofen can compound rhabdo’s kidney failure risk with their own kidney damage, especially when people take the pills to relieve soreness or stiffness associated with rhabdo.

Rhabdo also stands as a rebuttal to the conventional exercise wisdom of “no pain, no gain” or the urging to “feel the burn” that you hear from trainers. “ ‘No pain no gain’ is not anything you should ever follow,” said David Wang, a sports medicine physician at Hospital for Special Surgery, in New York, who specializes in the treatment of acute and overuse injuries and has served as a team physician for both college and professional athletes.

“We see [rhabdo] a lot in team settings, class settings, where people are trying to keep up with others, and they don’t want to look weak or that they can’t commit,” he said. “They’re not listening to themselves. They’re listening to other people.”

Wang is blunt about the dangers of trying to jump back into a pre-pandemic fitness routine. “It takes weeks and months to build up strength, and if you’re inactive for even just a week or two you lose all that strength and have to build it slowly back up again, so you’re not going to do what you were able to do a year ago or six months ago before the gyms closed.” Ignoring that reality, he said, is “the perfect setup to develop injuries, including rhabdo.”

 “When rhabdo happens, it’s an utter and complete shock. It’s an emotional shock. It’s a physical shock. People have never heard that exercise can be bad for them,” said Joe Cannon, a personal trainer who has a master’s degree in exercise science. “And I don’t even like saying that, because we live in a country where the vast majority of Americans do not exercise. And when people hear stories like this they think, ‘Oh my gosh! I shouldn’t even exercise because it could be bad for you.’ ”

For his part, Urban, whose 40 pull-ups hospitalized him with rhabdo, has continued working out with a self-described “slow progression” of slightly longer runs — a common rule of thumb is not to increase runs by more than 10 percent weekly — and a slow build of complementary push-ups and sit-ups. But not too many. “I got rhabdomyolysis because I was an idiot. My ego got in the way,” he said. “I learned my lesson.”
